USCIS Confirms Form I-693 Medical Examination Edition Date is Determined by Civil Surgeon’s Signature Date

As of Friday October 1, 2021, United States Citizenship and Immigration Services (USCIS) posted a new edition of Form I-693, the Report of Medical Examination and Vaccination Record.  This new version of Form I-693 requires all green card applicants to receive a full series vaccine for COVID-19. The new form is dated 9/13/2021 and as of October 1, 2021, USCIS will only accept the 9/13/2021 version of the I-693 Form. 

In its original notice, USCIS noted that beginning October 1, 2021, it would only accept the 09/13/2021 version. This raised concerns with the American Immigration Lawyers Association (AILA) which had clients that completed medical examinations prior to October 1st, 2021 but had yet to submit the certified Form I-693.  

USCIS clarified on October 4, 2021, that if the prior edition of Form I-693 was signed by a civil surgeon before October 1st, 2021, then USCIS would continue to accept that form after October 1, 2021.
